This Request for Proposals herein referred to as the “RFP” is soliciting applications to provide services in purpose built multi-unit properties used as transitional housing from homelessness with on site supports. The model will require oversights of the two (2) ICM Program Fixed Site locations located at 845 and 865 Davidson Avenue North in Listowel, Ontario. 845 Davidson Avenue North is comprised of 4 Units, each with 2 bedrooms. 865 Davidson Avenue North is comprised of 4 Units, each with 3 bedrooms. Congregate living with 100% occupancy would have the potential for up to 20 individuals, 8 families, or a mixture of both demographics in total for the buildings. There is an office space located at 845 Davidson that the successful RFP applicant will have use of for the duration of the contract to provide services to both fixed site locations. The model will require Housing Stability Workers on site a minimum of 35 operated hours a week. .
